FRAUD AFFIDAVIT

Frontline Asset Strategies takes claims of fraud or identity theft seriously.

If you believe that the account assigned to Frontline Asset Strategies was created without your knowledge or permission, or you were the victim of fraud or identity theft, which resulted in this account being created, please click on the FTC Fraud Form to open and download the required fraud affidavit. 

Once downloaded, please fill the fraud affidavit form completely and follow all instructions within the form. 

You may also choose to view additional information about the account by creating a user account and signing-in to www.payfrontline.com. Once signed in, you have the ability to view additional account details, communicate with us via email or chat, update contact information, request additional information, or one of the other available options.

For T-Mobile accounts: Please go directly to T-Mobile’s website at https://www.t-mobile.com/support/plans-features/help-with-scams-spam-and-fraud and scroll down to the section labeled T-Mobile account fraud. From there, click on the appropriate dispute form to submit all applicable details. Alternatively, you can download a version of the dispute form using the button below. Please do not return the completed fraud form to Frontline, rather follow the provided instructions for returning the form directly to T-Mobile at the address shown at the top of the form.

For Dish Network accounts: Please go directly to Dish Network’s website at https://www.dish.com/fraud/ and scroll down to the section labeled Identity Theft Claims. From there, click on the appropriate Dish Network Identity Theft Affidavit to download it, and follow the provided on-screen instructions for submitting the fraud claim. Alternatively, you can download a version of the identity theft affidavit, using the button below. Once you have completed the identity theft affidavit you may submit it directly to Dish Network at the same URL (https://www.dish.com/fraud/) by clicking the button labeled Begin Claims Process. If you prefer to mail your fraud claim along with supporting documentation, mail all documents to: Dish Network, L.L.C. Credit Team, 9601 S Meridian Blvd., Englewood, CO 80112

For Charter Spectrum accounts: Please go directly to Charter Spectrum’s website at https://www.spectrum.net/support/manage-account/reporting-id-theft, or in your preferred browser search engine search for “Spectrum ID Theft” to obtain the most up to date information for how to report your fraud claim. Alternatively, you can download a version of the Charter Spectrum Identity Theft form, using the button below. Once you have completed the identity theft form you may submit it directly to Charter Spectrum at the address provided in the downloaded identity theft form.
